In preparation for Thomasian Yearbook 2022, here are some reminders, and a checklist regarding the graduation photo.

Checklist

Answer the Health declaration form via THOMEDSS AND UPDATE YOUR COVID-19 IMMUNIZATION STATUS

We encourage our fellow Thomasian Scientists to answer the Health Declaration Checklists through the Thomasian Online Medical Services and Support (ThOMedSS) as often as possible.
 

Step 1. Go to thomedss.ust.edu.ph and login using your credentials in MyUSTe portal

 

Step 2. Click view case tracker under COVID-19 response

 

Step 3. Click the "new health declaration checklist for today" and answer the questions truthfully.

 

Step 4. Depending on the prompt, an action plan will be shown to you.

Together with this, kindly update your immunization records by UPLOADING your COVID-19 vaccination card. Follow these simple instructions:

Step 1. You will see the "Upload your COVID-19 Vaccination Card" right away in the homepage. 

Step 2. Click on the "Immunization" hyperlink. 


Step 3. Once redirected to the Immunization page, click on the "New Vaccination Record" to encode your COVID-19 vaccination details.

Step 4.. After encoding, click "Save".

Step 5. Upload your vaccination card under "Action" beside "Vaccination Card | Remarks" in the COVID-19 section.

Step 6. You may upload one photo instead if it contains all the doses you have taken for a vaccine.

 

You may also do medical consultations by clicking Online Consultation in the homepage.

Note: ThOMedSS is not for urgent medical cases. We suggest that you still reach the nearest medical facility for emergency concerns.

Answer the Student Regional Directory-Technological Capacity Survey

For an efficient understanding of the Technological Capacity of the whole College of Science student body, the CSSC would like to learn about your state and welfare.

 

The information gathered will be used for future basis of recommendations to the appropriate offices, suitable and capable to assist the student body in times of emergencies and/or calamities with technological concerns.
